Tracey Meise

Tracey joins us with over 25 years nursing experience with a focus on serving the senior population. She has spent many years in a variety of roles as a Registered nurse including long term care, assisted living, public health, home care and hospice. She has been a clinical nurse providing direct care to patients as well as more recently, providing medical oversight and organizational leadership in roles as health care manager and nursing director of medical adult day centers. Tracey has also taught certified nursing assistants as a clinical instructor and holds a certification as a Delegating Nurse in Maryland.

Tracey’s strengths are providing open, caring communication with clients and their families and providing care coordination of client care needs. Tracey greatly enjoys providing care in the home environment and building relationships with clients and families to assist in navigating their health care needs. She has worked directly with clients and families in their homes for many years and has the special ability to listen and create a plan of care to maximize safety while maintaining as much independence as possible.
